Familiengr\u00fcndung aufgrund des Risikos, die Krankheit an ihre Kinder weiterzugeben. Die Er\u00f6rterung von Fragen der Familienplanung mit einer genetischen Beratungsstelle kann Einzelpersonen dabei helfen, eine fundierte Entscheidung zu treffen. <\/p><p>F\u00fcr Patienten, die einen Kinderwunsch haben, gibt es mehrere diagnostische M\u00f6glichkeiten. Diese umfassen: <\/p><ul><li><strong>Pr\u00e4implantationsdiagnostik (auch bekannt als PID).<\/strong> Dies ist die Diagnose einer genetischen Erkrankung vor der Schwangerschaft. Diese Form der Untersuchung wird an den Eizellen einer Frau mittels In-vitro-Fertilisation durchgef\u00fchrt. Unbefruchtete Eizellen werden der Frau von einem Arzt entnommen und au\u00dferhalb der Geb\u00e4rmutter in einem Labor befruchtet. Die Embryonen werden im 6- bis 8-Zell-Stadium auf DM getestet. Nur nicht von DM betroffene befruchtete Eizellen werden in die Geb\u00e4rmutter implantiert.    <\/li><li><strong>Pr\u00e4nataldiagnostik.<\/strong> Eltern, die w\u00e4hrend der Schwangerschaft herausfinden m\u00f6chten, ob ihr F\u00f6tus das Gen f\u00fcr myotone Dystrophie geerbt hat, k\u00f6nnen sich einer pr\u00e4natalen Untersuchung unterziehen. Es stehen zwei Arten von Tests zur Verf\u00fcgung: <\/li><li><strong>Amniozentese.<\/strong> Bei diesem Verfahren wird eine Fruchtwasserprobe aus der Geb\u00e4rmutter entnommen, die vom F\u00f6tus abgesto\u00dfene Hautzellen enth\u00e4lt. Die Zellen werden dann im Labor gez\u00fcchtet, um DNA f\u00fcr Tests zu gewinnen. Der Test wird in der Regel in der 15. Schwangerschaftswoche durchgef\u00fchrt, und es kann 2 bis 3 Wochen dauern, bis die Ergebnisse vorliegen.  <\/li><li><strong>Chorionzottenbiopsie (CVS).<\/strong> Der Arzt entnimmt mit einer durch den Bauch oder die Vagina eingef\u00fchrten Nadel ein St\u00fcck Gewebe vom Rand der Plazenta. Das entnommene Gewebe enth\u00e4lt die gleichen genetischen Informationen wie der F\u00f6tus; die DNA wird isoliert und auf das Vorhandensein der Mutation der myotonen Dystrophie getestet. Der Test kann im ersten Trimester (in der Regel um die 10. Schwangerschaftswoche) durchgef\u00fchrt werden, und die Ergebnisse liegen normalerweise innerhalb von 1 bis 2 Wochen vor.  <\/li><\/ul><p>M\u00fctter mit DM1 sollten w\u00e4hrend der Schwangerschaft engmaschig \u00fcberwacht werden, da sie ein h\u00f6heres Risiko haben, ein Kind mit kongenitaler DM1 zu bekommen. In diesen F\u00e4llen kann sich \u00fcberm\u00e4\u00dfig viel Fruchtwasser (Hydramnion) ansammeln, was in der Regel bei einer Ultraschalluntersuchung festgestellt werden kann. H\u00e4ufig werden verminderte Kindsbewegungen festgestellt. Zudem k\u00f6nnen Beckenendlagen und schwache Wehen zu langen oder schwierigen Entbindungen f\u00fchren, die oft in einem Kaiserschnitt enden.   <\/p><p>Neugeborene mit kongenitaler myotoner Dystrophie ben\u00f6tigen sofortige intensivmedizinische Unterst\u00fctzung. Eine Entbindung in einem medizinischen Zentrum mit neonatologischer Unterst\u00fctzung f\u00fcr Hochrisikof\u00e4lle kann empfohlen werden. Unabh\u00e4ngig davon, ob Tests durchgef\u00fchrt werden oder nicht, sollten Personen mit einer famili\u00e4ren Vorgeschichte oder Symptomen einer myotonen Dystrophie ihren Geburtshelfer informieren,
