MDF’s Find a Doctor map is a community-built tool that helps you look for healthcare providers who people living with DM or their families have seen. This resource exists to help you explore options and start conversations about your care. MDF does not endorse any provider found on this list, and cannot guarantee the provider’s expertise or knowledge about DM care.
If you cannot find a provider near you, or you are unsure where to start, support is available.
1. Allow your location permissions
When prompted, select “Allow” or “Yes” when asked to view device location. If you selected “Don’t Allow” or “No”, don’t worry, follow next step which is to “Enter your location.”
2. Enter your location into the search field
Enter your location you want to search for the nearest facilities. You can move onto optional filters next or simply click on “Apply filters” to show all types of facilities.
3. (Optional) Filter by specialties
Select one or more specialties from the options available. Click on “Apply filters” when done.
4. (Optional) Filter by community recommendations
Select one or more recommendations from the options available. Click on “Apply filters” when done.
Some providers include community filters such as:
- Myotonic Dystrophy Type 1 (DM1)
- Myotonic Dystrophy Type 2 (DM2)
- Congenital Myotonic Dystrophy (CDM)
- Juvenile Onset (JOA)
These filters reflect that a person living with DM or a caregiver has shared their experience seeing that provider for a specific form of myotonic dystrophy.
These indicators are shared to provide helpful context. They do not mean that MDF recommends, endorses, or verifies a provider’s experience.
When choosing a provider, it may be helpful to ask about their experience with DM and whether they are open to learning more and using MDF’s Clinical Care Recommendations.
5. (Optional) Filter by badges
Select one or more badges from the options available. Click on “Apply filters” when done.

If a provider includes a community filter (such as DM1, DM2, CDM, or JOA), this means a person living with DM or a caregiver has shared their experience seeing that provider. These indicators do not reflect formal review, certification, or evaluation by MDF.
The database may change over time as community members submit new information. MDF may update or remove listings at any time. Use of this tool is voluntary and at your own discretion.
