2025 MDF Support Group Facilitator Application

Thank you for your interest in becoming a volunteer Support Group Facilitator (SGF) with the Myotonic Dystrophy Foundation (MDF)!

SGFs play a vital leadership role in offering support to the Myotonic Dystrophy (DM) community and connecting community members to MDF and its many resources. We are grateful for the dedication and commitment of our SGFs and look forward to reviewing your application.

What are SGF's?

SGFs are volunteer community leaders who receive training and support from MDF. Their role is to provide a place for those living with DM and their caregivers to confidentially share their experiences, learn from others also impacted by DM, and access resources and information to help improve quality of life. It is important to note that SGFs are not trained medical professionals and cannot give medical advice.

Support groups are offered in-person and/or virtually depending on the type of support group and the location. All of our upcoming virtual meetings are listed online.

SGF Qualifications & Responsibilities

  • SGFs must be a person with a genetic diagnosis or a caregiver of someone living with a genetic diagnosis of DM1 or DM2.
  • SGFs must have received their diagnosis at least one year prior to applying.
  • SGF applicatns must have experience attending MDF support groups.
  • SGFs must commit to hosting at least 4 virtual and/or in-person support groups each year.
  • SGFs must adhere to MDF guidelines and code of conduct.

Application Process

Interested community members may apply using the form below. Applications will be reviewed on a rolling basis.

All applicants must be available to attend the 2025 Virtual SGF training in October.

After completing their applications, all applicants will be connected with Lucie Shiffman, MDF’s Senior Manager of Community Engagement, for a conversation and interview regarding the position.

New SGFs will begin their role after participating in SGF training in October.

Section 2: Personal Profile

MDF is currently seeking co-facilitators for select support groups to ensure no SGF leads alone. Current opportunities include:

  • Caregivers for Children with Congenital Myotonic Dystrophy
  • DM2 Caregivers
  • Regional Groups: Michigan, Mountain West, NYC/NJ, Portland
  • SGF Substitute: to provide backup support across groups. This role requires flexibility and an additional interview with the Senior Manager of Community Engagement.

Section 3: Availability & Communication

SGFs are important members of the MDF team and the greater DM community. SGFs often are a friendly face that community members turn to with questions and to help them navigate their day-to-day journey.

Because this role is so vital to the well-being of the larger community, there are some requirements that must be met in order to become an SGF:

  • SGFs are required to host at least four meetings per year in person and/or virtually.
  • SGFs are required to participate in MDF-led training; this year, an SGF training session is being held virtually in October, 2025.
  • SGFs are required to attend a monthly 1-hour SGF-only administrative group meeting.
  • SGFs must participate in quarterly one on one meetings with the Sr. Manager of Community Engagement.
  • SGFs are asked, but not required, to attend the MDF Annual or Regional Conference each year.
  • Meetings must adhere to certain guidelines and procedures. If selected to become an SGF, please note these guidelines will be reviewed with you prior to your first meeting.
